On or about January 12, 2016, in the maintenance workshop, a 2Z341M Dayton Bench Grinder did not have a tongue guard attached, exposing employees to eye injuries, cuts or abrasions during the operating cycle.

On or about January 12, 2016, in the maintenance workshop, cylinders were stored throughout the shop not in designated areas, nor were they distinguished as empty or full, exposing employees to a safety hazard.
