29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    On or about January 13, 2016, employees on the steep roof of a newly built commercial building were exposed to a fall hazard exceeding 6 feet when performing roofing work without fall protection.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.      On or about January 13, 2016, employees on the roof of a newly built commercial building was exposed to a fall hazard when accessing the roof using a job-made wooden ladder without the side rails extending at least three feet above the landing area, secured, and a grasping device provided.
