29 CFR 1926.100(a):  Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:   (a) On or about January 13, 2016 at worksite 3000 Gillham Rd, Kansas City , Missouri.-employees were exposed to a struck by injury when working under elevated  materials and were not protected from overhead hazards by the use of a hard hat.

29 CFR 1926.501(b)(1):   Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    (a) On or about January 13, 2016 at worksite 3000 Gillham, Kansas City,Missouri.-employees were exposed to a fall hazard while performing framing activities at a height of approximately 50 feet without an approved form of fall protection to include guardrail systems, safety net systems, or personal fall arrest systems.
