29 CFR 1926.451(a)(6): Scaffolds were not constructed in accordance with the design by a qualified person: a) Inside the building, on or about January 14, 2016, two employees were elevated and working from an unapproved work platform that was supported by a Gradall JLG G6-42A #N19105 high reach forklift, in that it extended more than 69 inches outside the front load bearing tires on each side, it was missing toeboards, it did not have a slip resistant floor, and it did not have a load rating, which exposed employees to fall hazards up to 7 feet.

29 CFR 1926.451(c)(2)(v): The employer used forklifts to support scaffold platforms without ensuring that the entire platform was attached to the fork and that the forklift was not moved horizontally while the platform was occupied: a) Inside the building, on or about January 14, 2016, two employees were elevated and working from an unapproved work platform that was not secured to the forks of the Gradall JLG G6-42A #N19105 high reach forklift, which exposed employees to fall hazards up to 7 feet.

29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms without being specifically designed by the manufacturer for such use: a) East side of the building, on or about January 14, 2016, two employees were elevated and working from an unapproved work platform that was supported by a JLC high reach forklift 1044C-54 Series II, #412106185. The forklift manufacturer does not allow this forklift to be used to elevate personnel or be fitted with any form of personnel platform, which exposed employees to fall hazards up to 25 feet.
