29 CFR 1910.147(c)(4)(ii):  The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to the items of this section:    The employer failed to ensure that the lockout procedures used by employees to clean and service equipment were adequately specific for locking out equipment, including, but not limited to, #10 Braun Dryer.    Abatement certification is required for this violation.  The abatement certification sheet is enclosed with the citations.

29 CFR 1910.178(l)(4)(ii): The employer did not ensure that refresher training, including an evaluation of the effectiveness of that training, was conducted as required by paragraph (i)(4)(ii) to ensure that the operator has the knowledge and skills needed to operate the powered industrial truck safely in accordance with subpargragh [B]:    The employer failed to ensure that an employee was not allowed to operate a lift truck daily before being retrained and their performance being evaluated following an incident where an employee was injured while operating a lift truck.
