29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.  a. On line 1, employees operating the Kval 990FX machine were not guarded from the six-shooter heads moving up, down, and side to side, exposing employees to a struck by or caught in hazard.  b. On line 4, the Kval Handler, serial no. 07.02.388, did not have adequate guards in place to prevent employees from coming into contact with moving parts as the machine moved doors, exposing employees to an amputation hazard.

29 CFR 1910.147(c)(4)(ii): The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following:  29 CFR 1910.147(c)(4)(ii)(A): A specific statement of the intended use of the procedure.  a. In the production area, the procedures for the KVAL 990FX machine on line 1 did not have a statement to specify what the procedure was intended to be used for, such as but not limited to, a specific task to be performed or rendering the equipment to a zero state of energy.
