29 CFR 1926.453(b)(2)(v): A body belt shall be worn and a lanyard attached to the boom or basket when working from an aerial lift:    a. On or about January 12, 2016,  the employee working from the lift installing siding was not wearing a body belt and lanyard attached to the basket.  He was being exposed to a 13 feet fall hazard.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems;    a. On or about January 12, 2016, air conditioning platform, the employee fastening decking was not protected from falling 12 feet by the use of a guardrail system, safety net system, personal fall arrest system or any other type of fall protection method was being used.
