29 CFR 1910.147(c)(6)(i): The employer did not conduct periodic inspections of the energy control procedures at least annually to ensure that the procedures and the requirements of this standard are being followed:     (a)  BK Welding, at 244 Barberry Pl., Loveland, CO 80537:  On and before January 15, 2016, the employer did not conduct a periodic inspection of the energy control procedures at least annually to ensure that the procedure and the requirements of this standard are being followed.  The machine involved include such machines, but not limited to, the Controlled Automation Beam Line machines, the Piranha Ironworker, and the Detroit 5 Ton Crane.  Condition exposed employees to amputation hazards.    Abatement note:  29 CFR 1910.147(c)(6)(ii) provides that the employer shall certify that these periodic inspections have been performed.

29 CFR 1910.252(b)(2)(iii): Workers and other persons adjacent to the welding area were not protected from the rays by noncombustible or flameproof screens or shields:    a)  BK Welding, at 244 Barberry Place, Loveland, CO 80537:  On and before January 15, 2016, the employer did not ensure that welders and employees working near welders were protected from the welding rays.  The employer allowed up to six welders to work next to each other, but did not require the use of noncombustible or flameproof screens to serve as partitions, and did not require the use of protective eyewear at all times within these areas.  Condition exposed employees to eye injuries.

(a)  BK Welding, at 244 Barberry Place, Loveland, CO 80537: On and before January 15, 2016, the employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment.  Specifically, the employer had not conducted a hazard assessment to determine if the personal protective equipment which was being provided to their employees was adequate for the work tasks which they were assigned, such as welding, grinding, and the operation of cranes.

(a)  BK Welding, at 244 Barberry Place, Longmont, CO 80537:  On and before January 15, 2016, the employer failed to ensure that completed installations had a cover or faceplate.  Specifically, face plates was observed missing on a Ellis Sander. Condition exposed employees to potential electrical hazards.
