29 CFR 1910.132(d)(1)(i): The employer did not select and have each affected employee use, the types of personal protective equipment that would protect the affected employee(s) from the hazards identified in the hazard assessment:    Location: 6D Picking Platform    Employees are exposed to flash fires and thermal burns while removing items from conveyer without adequate thermal protective gear.      An employee was severely burned when the heat from the flash fire penetrated his uniform.

Location: 6D Picking Platform  Employees are exposed to flash fires while using a portable heater near a conveyer moving flammable materials.  An employee was severely burned when he was engulfed in a flash fire.
