29 CFR 1926.501(b)(13): Each employees engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):       (a-d) On the roof of apartment building numbers 2700, 2800, 2900, and 3800, east: On or about January 15, 2016, and at times prior thereto, employees were exposed to a fall hazard greater than 6 feet, while removing asphalt shingles from the roof.     (e) On the roof of apartment building number 2700, east: On or about January 15, 2016, and at times prior thereto, employees were exposed to a fall hazard, while walking across a 2x10 inch board from one building to another building to remove asphalt shingles from the roof.     (f) On the roof of apartment building number 2700, east: On or about January 15, 2016, and at times prior thereto, employees were exposed to a fall hazard, while jumping from one building to another building to remove asphalt shingles from the roof.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:     On the roof of apartment building numbers 2700, 2800, 2900, and 3800, east: On or about January15, 2016, and at times prior thereto, employees were exposed to a fall hazard, while removing asphalt shingles and jumping from building to building without being trained on fall hazard recognition.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:       (a) At the edge of apartment building number 2800, east: On or about January 15, 2016, and at times prior thereto, employees were exposed to a fall hazard greater than 6 feet, while using a 28 foot extension ladder to access the building roof that had the side rails approximately 14 inches beyond the landing, to remove and install asphalt shingles from the roof.       (b) At the edge of apartment building number 2900, east: On or about January 15, 2016, and at times prior thereto, employees were exposed to a fall hazard greater than 6 feet, while using a 28 foot extension ladder to access the building roof that had the side rails approximately 10 inches beyond the landing, to remove and install asphalt shingles from the roof.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards:   At the edge of apartment building number 2800 and 2900, east: On or about January 15, 2016, and at times prior thereto, employees were exposed to a fall hazard, while using a 28 foot extension ladder to access the building roof, to remove and install asphalt shingles from the roof without being trained on ladder usage.
