29 CFR 1910.179(j)(3): A complete periodic inspection of crane(s) had not been conducted in the past 12 months:    On or about January 12, 2016, and prior thereto, the employer had not completed a complete periodic inspection of the cranes in the past 12 months, exposing employees to struck-by hazards.  There were three instances:  1) In the work shop, on the southwest corner of the building there was a 2 ton Conco Crane (Dayton). 2) Out in the yard there was a Petty Bone 32 ton crane  3) Out in the yard there was a 15 ton Grove crane

On or about January 12, 2016, and at times prior thereto, the employer was storing the oxygen cylinders in the same location as the acetylene cylinders, exposing employees to struck-by and fire hazards.
