Employees on this site were exposed to hazards including, but not limited to fall and struck by hazards.  Condition noted on or about 01/12/16.  NOTE:  THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.1052(b)(1): Foot traffic was not prohibited on stairways where the treads and/or landings of pan stairs were to be filled in with concrete or other material at a later date and stairs were not temporarily fitted with wood or other solid material at least to the top edge of each pan.  a- 6th Floor Landing, 535 Flushing Avenue, Brooklyn NY: On or about 1/12/16 employees were exposed to trip and fall hazards when using landings of pan stairs that were not filled. These unfilled landings were located on the buildings 6th, 5th, 4rth, 3rd, and 2nd floors.   b- 5th Floor Landing, 535 Flushing Avenue, Brooklyn NY: On or about 1/12/16 employees were exposed to trip and fall hazards when using landings of pan stairs that were not filled.   c- 4rth Floor Landing, 535 Flushing Avenue, Brooklyn NY: On or about 1/12/16 employees were exposed to trip and fall hazards when using landings of pan stairs that were not filled.   d- 3rd Floor Landing, 535 Flushing Avenue, Brooklyn NY: On or about 1/12/16 employees were exposed to trip and fall hazards when using landings of pan stairs that were not filled.   e- 2nd Floor Landing, 535 Flushing Avenue, Brooklyn NY: On or about 1/12/16 employees were exposed to trip and fall hazards when using landings of pan stairs that were not filled.
