While conducting ship repair operations on Pier 1 for Motor Vessel Jean Mary, on or about January 14, 2016, in the Tool Storage area, four (4) outlet boxes had exposed live parts that were not guarded by use of approved faceplates or other forms of approved enclosures which did not prevent accidental contact with the bare and open wiring, conductors and terminal screws and exposed employees to electrical shock hazards.    b. While conducting ship repair operations on Pier 1 for Motor Vessel Jean Mary, on or about January 14, 2016, in the Carpenter Shops, one (1) outlet box had exposed live parts that were not guarded by use of approved faceplates or other forms of approved enclosures which did not prevent accidental contact with the bare and open wiring, conductors and terminal screws and exposed employees to electrical shock hazards.
