Section 5 (a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were being exposed to being struck by a front end loader bucket that had not been properly lowered prior to working on the vehicles hydraulic system.  Newark Paperboard Mill, 100 Newark Way, Fitchburg, Massachusetts:    On or about January 19, 2016 an employee was performing maintenance on a John Deere 624J front end loader to repair a hydraulic leak.  The employee had not lowered the bucket to the ground when securing the vehicle as required by the manufacturer.  This resulted in the rapid loss of hydraulic pressure and the bucket falling onto the employee's leg, crushing it.      Among other methods, feasible and acceptable methods to correct the hazards include but are not limited to: Ensuring that all required safety procedures in the manufacturers operations and service manuals are followed.     In addition to abatement certification, the employer is required to submit abatement documentation for this item in accordance with 29 CFR 1903.19.
