29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use: In the feed prep room, employees were required to mix, pour, and use chemicals including, but not limited to Argentyne to disinfect fish eggs, and the employer failed to ensure that suitable facilities for drenching or flushing the eyes were available in the immediate vicinity of the hazard. The eye wash provided was not suitable in that it had not been inspected and operated on a weekly basis.

29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective: In the break room, two GFCI outlets wired with an open ground. Employees using the break room were exposed to electric shock and fire hazards.
