29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    a) At the site, on the roof of the north east side of the house, on January 13, 2016, the employer did not ensure an employee who was installing decking on the roof, at a height of approximately twelve feet from the ground level, used a personal fall arrest system or other means of fall protection, exposing the employee to the hazards associated with, but not limited to death, paralysis, broken bones, lacerations, sprains, and bruises.

29 CFR 1926.1052(c)(1): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail and one stairrail system along each unprotected side or edge:    a) In the center of the house, on January 13, 2016, the employer did not ensure a flight of stairs consisting of seven risers, measuring a total of nine feet in height, used by employees to access the second floor of the house, was equipped with handrails, exposing employees to the hazards associated with, but not limited to broken bones, lacerations, sprains, and bruises.
