29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface, the ladder(s) was/were not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder: Along the east side of a 3-level residential building, on or about January 20, 2016, employees were exposed to fall hazards while performing roof shingle installation by accessing the top side roof using a Werner 24' aluminum extension ladder which was positioned over a sheet metal gutter system instead of a rigid section of the roof.
