29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface, the ladder(s) was/were not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:  Along the east side of a 3-level residential building, on or about January 20, 2016, employees were exposed to fall hazards while performing roof shingle installation by accessing the top side roof using a Louisville 28' aluminum extension ladder which was positioned over a sheet metal gutter system instead of a rigid section of the roof.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would train each employee in the procedures to be followed to minimize hazards related to ladders and stairways:  Along the top side of a 3 level residential building, on or about January 20, 2016, and at times prior thereto employees were not provided with training to recognize hazards related to use of ladders.
