The quad box was not listed or labeled for that use; on or about 1/19/16.  b) Worksite, 57 Spring St., New York, NY- Employees on the fourth floor were using a quad box lying on the floor to plug in a Makita cordless screwdriver and a Milwaukee cordless screwdriver., The quad box was not listed or labeled for that use; on or about 1/19/16.  Note:  The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.
