29 CFR 1926.403(i)(2)(i): Live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by cabinets or other forms of enclosures, nor by any of the means listed in subparagraphs (A) through (D) of this paragraph:    a)  At the jobsite, in the basement, on or about 1/20/16:  Employees were exposed to live and unguarded electrical equipment located at the newly installed service panel without a cover.      b)  At the jobsite, in the basement, on or about 1/20/16:  Employees were exposed to live and unguarded electrical equipment located at an additional unguarded panel.  This panel had exposed breakers and poorly spliced connections.        c)  At the jobsite, on the first floor, on or about 1/20/16:  Employees were exposed to live and unguarded electrical equipment located at an electrical panel without a cover.    d)  At the jobsite, in the stairwells, on or about 1/20/16:  Employees were exposed to live and unguarded electrical wiring where un-guarded lighting was hard-wired with exposed splices.
