29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) This violation occurred on or about January 20, 2016, and at times prior thereto; in the repair shop, where employees were exposed to burns hazards from being sprayed by solvent and/or hot water while working on the parts washer machines: a. Safety Interlock device on the door of the parts washer: Hayden Trans-Tool, model: #T-100HD-1, 061395966, 208/220 volts, 60 Amps, was missing/inoperative. b. Safety Interlock device on the door of the parts washer: Better Engineering, Jet washer, SN: #SC3975, was missing/inoperative.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws: (a) This violation occurred on or about January 20, 2016, and at times prior thereto; in the repair shop, where flexible cords to the drop lights did not have strain relief device causing a transmission of tension to joints and or terminals.
