29 CFR 1926.453(b)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift: Employees installing Tyvek home wrap were exposed to a fall hazard of approximately 12 feet while in the basket of an aerial lift and the employer did not ensure employees wore a suitable harness and lanyard attached to the basket.

29 CFR 1926.501(b)(14): Each employee working on, at, above, or near wall openings (including those with chutes attached), where the outside bottom edge of the wall opening was 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening was less than 39 inches (1.0 m) above the walking/working surface, was not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system: An employee was exposed to a fall hazard of approximately 16 feet while reaching his hands and arms out of the second story window openings to secure the tyvek home wrap and suitable fall protection was not used.
