The employer did not ensure unused opening were effectively closed or protected where employees were exposed to electrical shock and burn hazards:     a) On the south end of the storage building (Building 3), the metal gangway receptacle box mounted on the wall was missing a knockout and the cover was not securely fastened or missing;    b) On the north wall of the shipping building (Building 4), an circuit breaker panel cabinet used to supply power for the lights, ceilings fans, etc. had an opening that was not effectively closed; and    c) On an interior wall in the shipping building (Building 4), a Square D circuit breaker panel cabinet used to power shop lights, ceiling fan, a 220-amp plug, and an air compressor, had openings that were not effectively closed.
