29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent).  On or about 13 January 2016 and at times prior thereto, employees were working at a loading dock more than four feet above ground level which was not guarded with a guardrail, exposing the workers to the hazard of falling more than 4 feet to the ground below. Approximately 12 employees work daily at the West end, outside assembly area loading dock, which is approximately 52 inches above the ground below and not guarded with a standard guardrail or equivalent.

29 CFR 1910.157(g)(2): The educational program to familiarize employees with the general principles of fire extinguisher use and the hazards involved with incipient stage fire fighting was not provided to all employees upon initial employment, and at least annually thereafter.  On or about 13 January 2016 and at times prior thereto, the employer did not provide annual training to employees that were expected to utilize fire extinguishers to fight incipient stage fires, exposing the employees to fire and smoke hazards.

29 CFR 1910.252(b)(4)(iv): Where the welder must enter a confined space through a manhole or other small opening, means was not provided for quickly removing the welder in case of emergency.  On or about 13 January 2016 and at times prior thereto, employees were welding in confined spaces accessible through a vessel port hole without the means to quickly remove the welders, exposing the employees to the hazards of delayed rescue from a pipe/vessel up to 30 feet in length.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area.  On or about 13 January 2016 and at times prior thereto, the employer did not provide employees effective information and training on hazardous chemicals in their work area, exposing the employees to the hazards of working with hazardous chemicals without knowledge of the chemical hazards, protective equipment and exposure control, first aid measures, or handling and storage.
