29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms where the entire platform was not attached to the fork.  a)  On or about 01-25-16 at 6675 N Headwall Circle in Appleton, Wisconsin, an employee was working from an elevated platform supported by a forklift without securing the platform to the forks.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.  a)  On or about 01-25-16 at 6675 N Headwall Circle in Appleton, Wisconsin, the employer did not instruct his employee in scaffold safety such as fall hazards prior to working from an unsecured platform which was supported by a forklift.
