29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:  In the shop area, the Dayton 7 inch bench grinder was missing the tongue guard resulting in an approximate 1 inch gap between the abrasive wheel and the grinder housing, exposing employees to a struck by hazard.

29 CFR 1910.1200(g)(8): The employer did not maintain copies of the required material safety data sheets for each hazardous chemical in the workplace:  Employees were exposed to injury and illness from hazardous chemicals in that material safety data sheets were not available for all hazardous chemicals used in the facility. The employer did not have material safety data sheets for oxygen and acetylene in compressed gas cylinders.
