29 CFR 1910.134(k)(6): The basic advisory information on respirators, as presented in Appendix D of this section, shall be provided by the employer in any written or oral format, to employees who wear respirators when such use is not required by this section or by the employer. (a) TNG-DG at 11111 E. 53rd Avenue, Denver, CO: On and before 01/26/16, the employer allowed for the voluntary use of Sperian Saf-T-Fit Plus N1115 White N95 Molded Cup Respirator to employees without providing basic advisory information contained in Appendix D of this section. The use of filtering face-piece half mask respirators without knowledge of manufacturer instructions regarding respirator limitations, proper storage, use, and cleaning of the respirator could expose employees to health hazards from respirable dust.

29 CFR 1910.1200(e)(1): Employers shall develop, implement, and maintain at each workplace, a written hazard communication program which at least describes how the criteria specified in paragraphs (f), (g), and (h) of this section for labels and other forms of warning, material safety data sheets, and employee information and training will be met: (a) TNG-DG at 11111 E. 53rd Avenue, Denver, CO: On and before 01/26/16, the employer did not ensure that a written hazard communication program was developed and implemented for the establishment. This condition exposed employees to chemical hazards associated with, but not limited to, use of chemicals listed below: 1. Carbon monoxide gas; employees may be exposed to carbon monoxide gas due to the operation of propane powered forklifts in the work area. Possible hazards associated with use of this product include respiratory distress and asphyxiation.
