29 CFR 1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment were not independent of any anchorage being used to support or suspend platforms and/or were not capable of supporting at least 5,000 pounds (22.2 kN) per employee:  On or about 1/26/2016, during demolition activities, the employer failed to ensure that anchorage points would support at least 5,000 pounds per employee. The 2 employees were anchored to a Genie S-45, Serial number S4506-11472, which was rated for a static load of only 500 pounds.
