29 CFR 1926.452(w)(2): Scaffold casters and wheels were not locked with positive wheel and/or wheel and swivel locks, or equivalent means, to prevent movement of the scaffold while the scaffold was used in a stationary manner:  (a) Jobsite- 7th floor, 400 Stanwix Street, Pittsburgh, PA: On or about January 27, 2016, employees using the bakers scaffold did not have scaffold wheels locked when it was being used in a stationary manner.
