29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:   (a) Roof, Autumn Leaves Memory Care Center located at 3300 Webb Bridge Road, Alpharetta, GA 30005: Employees were exposed to eye injuries while operating pneumatic roofing nail guns at 120 PSI without protective eyewear. Employees were exposed to struck-by hazards which included 1-1/4-inch fasteners and metal fragments from the collated coil nails.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  (a) Roof, Autumn Leaves Memory Center located at 3300 Webb Bridge Road, Alpharetta, GA 30005: Employees were exposed to fall hazards of approximately 11-feet, 2-inches while engaged in roof activities on a 6/12 pitched roof with no fall protection system in place.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  (a) Roof, Autumn Leaves Memory Care Center located at 3300 Webb Bridge Road, Alpharetta, GA 30005: The employer failed to provide fall protection training to employees performing roofing activities on a steep roof at height of approximately 11-feet, 2-inches from the lower level. Employees were not provided a fall protection system or trained to recognize a fall hazard existed.
