29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    a) Exterior and Interior of Building #5 - 2nd Deck: Employees without any means of fall protection were exposed to a fall hazard of approximately 20 feet while engaged in framing activities, on or about 11/4/15.

29 CFR 1926.1051(a): Stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided:   a) Access from ground level to 2nd floor: Employees were accessing the second floor from the ground level by climbing the frame walls, being exposed to fall hazards of approximately 20 feet, on or about 11/4/15.
