29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:  Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.  The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated   (a) Rotunda - On or about January 27, 2016 two employees were exposed a fall hazard while riding in a Genie Z-40/23N Boom Lift (serial no, HR3004). Employees were elevated 31 feet 3 inches above concrete without body belt or body harness.

29 CFR 1926.1052(c)(1): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail and one stairrail system along each unprotected side or edge:  (a)  Rotunda Staircase - On or about January 27, 2016 employees were exposed to a fall hazard while walking up and down an open stairway with no handrail installed. The stairs highest point was 17.6 feet above concrete.
