29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: JOB SITE: 4055 Veterans Memorial Parkway, St. Peters, MO 63376, on or about 1/28/2016, in front of the building, two workers were observed on a Genie aerial lift, actively doing plastering work, at height greater than 10' 5" above ground level and were not protected from falling.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of aerial lift being used and to understand the procedures to control or minimize those hazards: JOB SITE: 4055 Veterans Memorial Parkway, St. Peters, MO 63376, on or about 1/28/2016, the employer failed to trained by a person qualified in the subject matter to recognize the hazards associated with the type of aerial lift being used and to understand the procedures to control or minimize those hazards. The workers were on a Genie aerial lift in an elevated height greater 10' 5" above ground level and were not tied down with a lanyard to an anchor point on the aerial lift.
