29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  (a) West side, new residential construction located at 3528 Strath Drive, Alpharetta, GA 30005: Employees operating pneumatic finish, framing, and siding nail guns were exposed to eye injuries while operating these tools at 120 PSI with no protective eyewear. Employees were exposed to collated debris and fasteners as safety glasses and/or goggles were not worn.

29 CFR 1926.451(b)(7): On scaffolds where platforms are overlapped to create a long platform, the overlap did not occur only over supports:  (a) West side, new residential construction located at 3528 Strath Drive, Alpharetta, GA 30005: Employees were exposed to potential collapse/fall hazards of approximately 19-feet, 4-inches to the lower level while working from a pump jack/ladder jack scaffold system which did not have the work planks bridged over a support pole. In the locations where the bridging occurred there was no supporting pole which created a fall hazard due to potential collapse.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used:  (a) West side, new residential home located at 3528 Strath Drive, Alpharetta, GA 30005: Employees were not provided with a safe means of access to the work platform. Employees used the ladder jack support ladders to access the working level and the lower level and were exposed to falls of approximately 19-feet, 4-inches to the ground below.
