29 CFR 1910.23(a)(4): Every skylight floor opening and hole was not guarded by a standard skylight screen or a fixed standard railing on all exposed sides.   (a) On or about October 28,2015, Western Slope Plumbing & Welding, Inc. did not ensure that skylight openings and holes were guarded by a standard skylight screen or a fixed standard railing on all exposed sides. An employee of Western Slope Plumbing & Welding, Inc. was seriously injured when he stepped onto an unprotected skylight panel, falling approximately 20 feet to the ground below.  Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:    (a) On or about October 28, 2015, Western Slope Plumbing & Welding, Inc. did not ensure that protective equipment was used when necessary whenever hazards capable of causing injury were encountered. Employees were exposed to fall hazards of up to 20 feet while climbing a radio tower to gain access to a building roof top, employees did not use any protective equipment.
