29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:    a.  Bridgetown Subdivision, Lot #412, 3545 Golden Gate Lane, Cumming, GA (Jobsite)- Employees were exposed to struck-by hazard.  Employees were in the process of cutting hardi-plank siding, for the installation to the exterior residential home, without the use of eye protection, on or about 01/28/16.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501:      a.  Bridgetown Subdivision, Lot #412, 3545 Golden Gate Lane, Cumming, GA (Jobsite) - Employees were exposed to fall hazard.  The employees were not utilizing a means of fall protection while working and walking approximately 14 feet above ground level on the roof, Installing exterior siding along the front of the residential home while walking and standing on the upper level roof at various areas which a slope pitch of 4-10:12, on or about 01/28/16.
