OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fire hazards during heating or drying of plastics in ovens or thermoforming tables:     a. On or about January 27, 2016 and at times prior thereto, in the shop, employees heated plastic sheets at a thermoforming table in which the heating elements are below the level of the plastic sheet that is being heated.  Adequate protection to prevent the plastic sheet from contacting the heating elements and causing a fire was not provided.       b. On or about January 27, 2016 and at times prior thereto, in the shop, employees heated plastic at thermoforming tables and in a drying oven that lacked excess temperature limit controllers which exposes employees to fire hazards.       c. On or about January 27, 2016 and at times prior thereto, in the shop, employees heated plastic in a drying oven that lacked instructions for normal operations and emergency operations which exposes employees to fire hazards.       d. On or about January 27, 2016 and at times prior thereto, in the shop, employees heated plastic in a drying oven which did not have an exhaust duct that vented directly to the outside of the building which exposes employees to chemical hazards including but not limited to carbon monoxide.
