29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  On or about January 25, 2016:  a) Employees working on installing truss bracing were exposed to head injuries while working below another employee installing more bracing when the employer failed to ensure that employees wearing proper head protection while performing these tasks.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.  On or about January 25, 2016:  a) Employees installing roof truss bracing on a new residential structure were exposed to eye injures when the employer failed to provide safety glasses when the employees were using pneumatic nail guns to perform tasks.  b) An employee was exposed to eye injuries when using a circular saw to preform cutting task when the employer failed to provided safety glasses.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:  On or about January 25, 2016:  a) An employee operating a Makita circular saw was exposed to electrical hazards when the flexible cord used to power the saw did not have proper strain relief on the cord.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about January 25, 2016:  a) An employee installing bracing into trusses on a new residential strucuture was exposed to fall hazards of approximately 57 feet when the employer failed to ensure that the employee was given proper fall protection measures.  b) Employees installing bracing on a new residential structure were exposed to fall hazards of approximately 9 feet while standing on the top plate area of the second floor. The employer failed to ensure that the employees were adequately protected by proper fall protection means.  Chico Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 982629, citation number 1, item number 1 and was affirmed as a final order on September 8, 2014, with respect to a workplace located at 2062 Stonewater Creek Roadlot #11a Hoschton, GA 30548.
