29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use: On or about January 27, 2016, on the east side of a newly built commercial building, an employee was exposed to a fall hazard exceeding 10 feet when performing work from the forks of a JLG rough terrain forklift, Model 6042, SN 10333379, without the work platform being designed by the manufacturer for such use.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section. On or about January 27, 2016, on the east side of a newly built commercial building, an employee was exposed to a fall hazard exceeding 10 feet when performing work from the forks of a JLG rough terrain forklift, Model 6042, SN 10333379, without a fall protection system.
