29 CFR 1910.22(b)(1):  The storage of material shall created a hazard. Items such as bags, containers, bundles, etc., were not stored in tiers and stacked, blocked, interlocked and limited in height so that they were stable and secure against sliding or collapse:    a)   At the facility, on or about December 15, 2015 the storage of approximately 18 tubes on a portable yard trailer created a hazard in that the load was not properly secured on the trailer to sliding off the back of the trailer.  The approximately 18 tubes, weighing approximately 44000 lbs. were secured by approximately 2 chains.  Storing the tubes on the trailer in this manner caused the tubes to slide off the back of the trailer.    The Portersville Sales & Testing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.176(b), which was contained in OSHA inspection number 1091963, citation number 1, item number 3 and was affirmed as a final order on 12/15/15, with respect to a workplace located at 1406 Perry Hwy, Portersville, PA 16051.
