29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: a) Throughout Jobsite - On or about January 29, 2016, employees were working from aerial lifts (e.g. self-propelled telescopic booms) without the use of fall protection, exposing the employee to fall hazards of up to about 17 feet onto concrete and earthen ground.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: a) Bleachers - On or about January 29, 2016, the employer did not ensure that employees were fall protected while walking/working at the edge of the bleacher seats with sides that were unprotected by guardrail systems or safety net systems, exposing employees to fall hazards up to about 20 feet onto concrete and earthen ground. Bleachers Builders, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(1), which was contained in OSHA inspection number 1057449, citation number 1, item number 1 and was affirmed as a final order on June 10, 2015, with respect to a workplace located at 1801 W International Speedway Blvd, Daytona Beach, FL 32114.
