29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:    (a)  Calco Concrete Pumping, Inc., at 100 Vail Road, Vail, CO 81657:   On and before January 29, 2016, the exposing employer failed to conduct a job hazard analysis and to instruct each employee to stand back from the hose reducer connection points of the concrete pumper while priming the pumping system.  Condition exposed employees working in close proximity to concrete pumping operations to the hazards of being struck by rock, sand, cement, and concrete, and to fall hazards associated with uneven terrain.

29 CFR 1926.701(f):  Employees were allowed to apply a cement, sand, and water mixture through a pneumatic hose without the use of protective head and face equipment:    (a) Calco Concrete Pumping, Inc. at 100 Vail Road, Vail, CO 81657; On and before January 29, 2016, the exposing employer failed to ensure that employees applying a cement, sand, and water mixture when priming the concrete pumper wore protective face equipment such as goggles or face shields.  In addition, employees were allowed to apply concrete without the use of protective face equipment such as goggles or face shields.  Condition exposed employees to the hazard being struck by these materials, causing eye and face injuries.
