29 CFR 1926.20(b)(2): Such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers.    Location - 50 Humphrey Street, Swampscott, MA  On or About January 28, 2016, a competent person did not perform a frequent and regular inspections of the jobsite to identify safety and health hazards such as but not limited to:  employees were working from a roof at height greater than 25 feet above ground level while wearing incorrect fall protection systems, employees were not protected from falling while working from a roof greater than 25 feet above ground level, employees were using pneumatic nail guns without proper eye protection and an employee working below roof work did not have head protection to protect him from falling roof debris.

29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets.    Location - 50 Humphrey Street, Swampscott, MA  On or about January 28, 2016, an employee in a roll-off was cleaning up roofing debris that was being tossed down from the roof to the ground. Employee had no head protection.
