29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes by covers.  On or about January 28, 2016, at or near The Greens at Longhills Apartments, 327 Hwy 5N, Benton, AR 72019, an affected employee who was moving plywood from a deck tripped in a hole on the floor. This condition exposed employees to trip and fall from height hazards.

29 CFR 1926.501(b)(13): Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system or personal fall arrest systems:  On or about January 28, 2016, at or near 327 Hwy 5N, Benton, AR 72019, an affected employee who was moving plywood from a deck above 6ft was not protected from falling by guardrail, safety net, or personal fall arrest systems. This condition exposed employees to fall from height hazards.
