On or about January 22, 2016 Employees operating a Makita Brand 4-1/2" right angle grinders/cut off wheel to trim or cut ceramic floor tiles were not protected from the hazard of flying, abrasive, and splashing objects, because the manufacturer supplied guard for the cut off wheel was not being used on the machine.
