The employer is failing to protect employees from fall hazards when they are working in the vicinity of a ladder system and floor hole.  This was most recently documented at 1850 P Street, Lincoln NE, 2nd Floor, where employees were observed in the area of guardrail systems constructed without mid rails.  The guardrails were installed around ladder access points and constructed with lumber (2X4) consisting of top-rails and vertical members.  The floor holes each had falls to the next lower level of over 9 feet.         a.  2nd floor East Ladder access point; mid-rails were not installed.                                                                                              b.  2nd floor West Ladder access point; mid-rails were not installed.         Abatement certification is required for this violation.  The abatement certification sheet is enclosed with the citations.
