29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms without being specifically designed by the manufacturer for such use:   On or about February 3, 2016, at the above addressed site, an employee performing carpentry work exposed to a fall hazard when working from a platform supported by the forks of a forklift that was not specifically designed by the manufacturer for such use.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.  On or about February 3, 2016, at the above addressed site, an employee performing carpentry work exposed to a fall hazard while on a work platform supported by a forklift approximately 12 feet 4 inches in height was not provided with a training program for him to recognize the hazards associated with falls.
