29 CFR 1926.451(g)(1)(ii): Each employee on a single-point or two-point adjustable suspension scaffold was not protected by both a personal fall arrest system and a guardrail system.  Site: 124 East 107th Street New York, NY  On or about 2/4/16  a) Employee working from a two point C hook suspended scaffold was exposed to a fall from approximately 70 feet high. The guardrail system was incomplete, it was missing the midrail.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  Site: 124 East 107th Street New York, NY  On or about 2/4/16  a) Company owner was exposed to fall hazards from approximately 70 feet. He was working on the 7th floor from the fire escape area of the building without fall protection.
