At the site, employees were exposed to a fall hazard, in that, two mobile scaffolds two frames high, were not provided with a cross brace at the one side of the top frame. A 6 inch wide plank was used to bridge the mobile scaffolds without been braced or otherwise secured in any way from displacement while in use, on or about 02/04/2016.
