29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees.    On or about January 4, 2016, in the shop area, lockout devices were not affixed to energy sources on the table saws where an employee was performing servicing and maintaining, thereby exposing the employee to hazards associated with the unexpected startup of machinery.

29 CFR 1910.176(b): Secure storage. Storage of material did create a hazard. Bags, containers, bundles, etc., stored in tiers were not stacked, blocked, interlocked and limited in height so that they are stable and secure against sliding or collapse.    On or about February 1, 2016, employees were handling eight 4x8 pieces of stored glass and the material was not stable and secure to prevent from falling over. Employees were thereby exposed to struck by hazards.
